Differential diagnosis of oncocytic pleomorphic adenoma |

I read with great interest the recent case report by Mariano et al. [1] The authors report a unique case of oncocytic pleomorphic adenoma (PA) of the parotid gland. However, the authors have omitted an important differential diagnosis, namely, oncocytic epithelial-myoepithelial carcinoma (EMC).
Oncocytic EMC was a recently described variant of EMC and this entity should be included in the differential diagnosis of oncocytic tumors of salivary glands. [2],[3],[4] With reference to this case reported by Mariano et al., [1] a biphasic nature of the tumor was confirmed by histopathology and immunohistochemistry. Both oncocytic PA and oncocytic EMC should be considered. Distinguishing features from PA include documentation of invasion and the absence of chondromyxoid stroma in EMC. However, this case was devoid of chondromyxoid stroma, which was usually seen in previously reported oncocytic PA. [5] Although an evident capsule was noted by the authors, it should be kept in mind that at least partial encapsulation in EMC is relatively common and complete encapsulation with minimal invasion was seen in 19.7% EMCs. [3] Therefore, a careful examination of invasion is required to exclude oncocytic EMC before a diagnosis oncocytic PA is made.
