   Brief Clinical History Top

A 50-year-old female presented with pain in her lower back, including her left hip of 6 years' duration. She had undergone a hip replacement. A year ago, she had a fall and was treated for hairline fractures of right femur and fourth and fifth metatarsals of her left foot. She had an incidental phyllodes tumor in her breast and a soft-tissue lesion in the web space between the third and fourth toes of her right foot, measuring 0.8 cm × 1.4 cm × 1.0 cm. Gallium-68 DOTATATE scan showed a focal high-grade somatostatin receptor expression in the soft-tissue lesion.

Biochemical findings

Her serum calcium level was 8.59 mg/dl (normal range = 8.8–10.2); serum phosphorus level was 0.82 mg/dl (normal range = 2–5.5). Serum alkaline phosphatase level was 194.52 U/L (35–105) and serum parathormone level was 154.40 pg/ml (normal range = 115–68.3).
